It's used in two ways: to print white elements (text, logos, highlights) on dark fabric, and as an "underbase" — a hidden white layer printed first so colours placed on top read true instead of sinking into the dark shirt. DTF, screen and (with pre-treatment) DTG all handle white ink well.
